Use a blue or black ink pen with legible handwriting. All information has to be identical to the verified information you provided on the Sixty6 site when creating your User Account.
Request the unique Postal Request Code by following these steps:
- Log in to your Account;
- Go to the ‘Sweepstakes Rules’ page;
- Find the AMOE section;
- Click on the ‘Postal Request Code’ link;
- Follow the instructions that appear;
