You can use blue or black ink, and your handwriting must be legible. All information must be identical to the verified information you provided on the casino’s site when creating your User Account.

We have all the information you need to send the Mail-in Request to Sportzino’s AMOE(1).

What’s Needed For Sportzino Mail-in Requests

  • A verified Sportzino account;
  • Stamped #10 Envelope;
  • A blank, standard card;
  • A blue or black ink pen.

What To Write on the Piece of Letter or Piece of Paper

On the Request Card, handwrite:

  • Your full name as displayed on your government-issued identification;
  • The e-mail address registered on your Sportzino Account;
  • Your return address registered on your Sportzino Account;
  • Your Sportzino user ID;
  • The following statement:

FAQ Sportzino Mail-In Request

Are There Any Mailing Restrictions?

Yes, the request must be mailed from the same state as the return address registered to your account. Only one request per envelope is allowed.

Can I Type or Electronically Generate the Request?

No. The request must be handwritten, and it cannot be created electronically, nor generated by autopen or similar tools. If there is suspicion that the request was not handwritten by you, it may be denied at Sportzino’s sole discretion.

Do I Need to Complete Identity Verification (KYC)?

Yes. To comply with policies and legal requirements, you must complete applicable KYC (Know Your Customer) verification before you are eligible to redeem or request any free rewards.
